A: Standard appointments are 15 minutes. New patients are recommended to have a double i.e. 30 minute initial appointment. You can help us run to time by emailing to reception the New Patient form in advance, so that your file can be made ready before your first consultation. I also recommend that a double appointment is booked for mental health, complex problems, or if you have more than one issue.
Q: How do I get my notes transferred to your new clinic?
A: Please ask Reception for a Transfer of Record Form which they can fax to your previous clinic with your signed consent. Please note that it is a good idea to do that as soon as you can, as clinics are only legally obliged to hold onto patients' records until the patient is 25, or a maximum of seven years after last contact (whichever is longer), after which the notes may be destroyed.
Q: What is My Health Record?
A: The Australian Government is encouraging all patients to register for My Health Record, an online summary of your health information. At present, this is limited to a summary of major medical diagnoses, medications, allergies and immunisations. There is an opt out period if you do not wish to have a record automatically created for you. Check back with Department of Health as this advice is continually being reviewed.
